Testosterone suppresses hepcidin

Hepcidin is the hormone that locks iron away. In a 20-week dosing study, high testosterone levels markedly suppressed serum hepcidin within one week, and the suppression was dose-dependent and stronger in older men (PMID 20660052). Hepcidin limits iron release from the gut and storage cells (PMID 42700347), so less hepcidin frees more iron for use. The effect is a suppression, not a permanent off-switch: in older men on testosterone gel, hepcidin fell at 1 and 3 months but returned toward baseline by 6 months despite continued testosterone (PMID 24158761).

A 2026 review of six studies covering 480,825 adults found GLP-1 RA users had 26–30% lower ferritin than SGLT2 inhibitor users, and more than 60% of users ate below estimated iron requirements. Ferritin is not a pure iron gauge. In 628 biopsy-proven NAFLD patients, ferritin above 1.5× the upper limit of normal was tied to NASH and more advanced fibrosis (PMID 21953442). In obesity, inflammation drives hepcidin up, and weight loss appears to lower inflammation and hepcidin (PMID 42700347). Whether GLP-1 ferritin drops reflect this has not been shown in humans (PMID 42559859).

Is it depletion or a marker shift?

Ferritin alone cannot answer this. A 2026 review of obesity and iron metabolism recommends reading ferritin next to an inflammation marker such as CRP, supported by transferrin saturation, soluble transferrin receptor or reticulocyte hemoglobin (PMID 42700347). A 2026 hematology review of the diagnostic approach to abnormal ferritin adds that the sTfR/log-ferritin index helps separate iron deficiency from anemia of inflammation, although sTfR is itself influenced by inflammation (PMID 42559859).
