Retatrutide & Kidney Disease: What the 54% Really Means
A viral post says retatrutide cut urine protein 54% in people with damaged kidneys. The number is real, but it comes from a subgroup of a 146-person mechanistic trial whose primary endpoint (measured kidney filtration) did not change, and it has not been peer-reviewed yet.

Four things the 54% headline leaves out
Per HCPLive's report of ASN Kidney Week 2026 poster TH-PO1238 (Oct 22, 2026), urine albumin-to-creatinine ratio (UACR) fell 53.7% on retatrutide vs 8.8% on placebo, a placebo-adjusted difference of −49.2% (95% CI −70.6% to −12.3%). That figure is for participants who started with UACR ≥30 mg/g, about 30% of the trial. The whole trial's median baseline UACR was 14.0 mg/g (PMID 41160422).
TRANSCEND-CKD was built to test measured GFR by iohexol clearance at week 24 (PMID 41160422). In the same conference report, measured GFR changed −1.6 vs −0.2 mL/min/1.73 m² (difference −1.4; 95% CI −9.2 to 6.3), which did not differ between groups. Creatinine- and cystatin C-based eGFR estimates both rose vs placebo.
The trial used MRI to measure kidney blood flow and fat around the kidney (PMID 41160422). The poster reported perirenal fat volume −16.1% vs −2.7%, renal sinus fat −5.8% vs +0.7%, and estimated intraglomerular pressure −2.8 mm Hg vs placebo. Weight fell 14.0 kg (about 31 lb) and systolic blood pressure fell 14 vs 1 mm Hg.
TRANSCEND-CKD is a 24-week mechanistic study meant to inform the separate cardio-kidney outcome trial TRIUMPH-Outcomes, NCT06383390 (PMID 41160422). Albuminuria is a lab marker, not dialysis or kidney failure. For comparison, semaglutide reduced a kidney-outcome composite by 24% over a median 3.4 years in the FLOW trial (PMID 38785209). Retatrutide has no equivalent result yet. Pulse rose about 9 bpm on retatrutide in the poster.
What the Data Shows
Key Takeaways
- TRANSCEND-CKD randomized 146 adults with overweight or obesity and CKD (eGFR 25-75), with or without type 2 diabetes, 1:1 to retatrutide up to 12 mg or placebo (PMID 41160422).
- In the roughly 30% of participants with UACR of 30 mg/g or more, UACR fell 53.7% on retatrutide vs 8.8% on placebo at 24 weeks (ASN Kidney Week 2026 poster, as reported by HCPLive).
- The primary endpoint, measured GFR by iohexol clearance, did not differ from placebo at 24 weeks.
- In earlier phase 2 trials of people with mostly normal albuminuria, retatrutide 12 mg lowered UACR 37.0% (type 2 diabetes, 36 weeks) and 31.5% (obesity, 48 weeks) vs placebo (PMID 40630318).
- Weight fell about 14 kg and systolic blood pressure fell about 13 mm Hg vs placebo in TRANSCEND-CKD, while pulse rose about 9 bpm.
- In mouse models of diabetic kidney disease and kidney fibrosis, retatrutide reduced inflammatory and fibrotic markers (PMID 39212900, PMID 42630988).
- Whether retatrutide prevents kidney failure, dialysis, or kidney death. That is the job of TRIUMPH-Outcomes, which has not read out.
- The full peer-reviewed TRANSCEND-CKD results. The 24-week numbers come from a conference poster.
- How much of the albuminuria drop is due to weight loss and lower blood pressure versus a direct kidney effect.
- Whether the effect holds past 24 weeks, or in people with eGFR below 25.
- What the TRANSCEND-CKD adverse-event profile looked like; the conference report did not give it.
- Whether the animal-model anti-fibrosis findings translate to people.
Frequently Asked Questions
Did retatrutide really cut urine protein by 54% in kidney disease?
Close, with context. In the phase 2b TRANSCEND-CKD trial, urine albumin-to-creatinine ratio (UACR) fell 53.7% on retatrutide vs 8.8% on placebo at 24 weeks, a placebo-adjusted drop of 49.2%. That figure covers only participants who started with UACR of 30 mg/g or more, about 30% of the 146 randomized. It comes from an ASN Kidney Week 2026 poster, not a peer-reviewed paper yet.
Did retatrutide improve kidney function in TRANSCEND-CKD?
Not on the primary endpoint. Measured GFR by iohexol clearance changed −1.6 vs −0.2 mL/min/1.73 m² at 24 weeks, which did not differ from placebo. Creatinine- and cystatin C-based eGFR estimates rose vs placebo, and MRI showed less fat around the kidneys and lower estimated intraglomerular pressure. These are markers, not kidney outcomes.
Who was in the retatrutide kidney trial?
TRANSCEND-CKD enrolled adults with overweight or obesity and chronic kidney disease (eGFR 25 to 75 mL/min/1.73 m²), with or without type 2 diabetes. Of 146 randomized, mean age was 65.1 years, mean BMI 35.7, 37.7% had type 2 diabetes, mean measured GFR was 49.3, and 21.8% were already taking an SGLT2 inhibitor (PMID 41160422).
How does retatrutide compare with semaglutide for kidney disease?
They are at different stages. Semaglutide has a completed kidney-outcome trial: in FLOW, it lowered the risk of a kidney-failure, eGFR-decline, or kidney/cardiovascular-death composite by 24% over a median 3.4 years in people with type 2 diabetes and CKD (PMID 38785209). Retatrutide so far has only 24-week marker data in CKD; its outcome trial, TRIUMPH-Outcomes, is ongoing.
Did retatrutide lower albuminuria in earlier trials?
Yes, modestly. A post hoc analysis of the two phase 2 trials, in type 2 diabetes (PMID 37385280) and in obesity (PMID 37366315), found retatrutide 12 mg lowered UACR 37.0% vs placebo at 36 weeks in type 2 diabetes and 31.5% at 48 weeks in obesity. Because most participants had normal albuminuria at baseline, the absolute reduction was small (PMID 40630318).
