Zolucatetide: The FAP Polyp Readout, in Context

📄 7 PubMed citations

Posts say a new peptide reduced inherited cancer polyps in a Phase 1 study. That is not the whole story: the data are company-reported results from two patients in an open-label Phase 1/2 trial, with no peer-reviewed paper yet.

🔬 HighPeptides went past the viral post to the primary records: the ClinicalTrials.gov entry (NCT05919264), Parabilis Medicines' own press releases, and the peer-reviewed FAP chemoprevention trials that earlier drugs went through. As of October 16, 2026, PubMed has no paper on zolucatetide. Everything below is labelled by where it comes from.

How It Works

🧬
Why FAP drives polyps

FAP is caused by germline pathogenic variants in APC, a gene in the Wnt signalling pathway. Most cancer-causing Wnt defects raise cellular β-catenin by blocking its normal breakdown, and β-catenin then acts as a transcriptional coactivator for TCF proteins.

🎯
The target: β-catenin:TCF

Parabilis describes zolucatetide as the first direct inhibitor of the β-catenin:TCF interaction. This protein-protein contact has resisted small-molecule drugs. A 2012 PNAS study showed a hydrocarbon-stapled peptide could bind β-catenin directly and interfere with its coactivator role for TCF.

🌀
What a Helicon peptide is

Helicons are peptides locked into an α-helix. FOG Pharmaceuticals scientists describe them as able to enter cells and bind targets long considered undruggable, such as protein-protein interactions. Their 2022 PNAS paper reports a phage-display method for finding such helical binders.

💉
How it is given in the trial

On ClinicalTrials.gov the drug appears under its code FOG-001, given by IV or subcutaneous injection in continuous 28-day cycles. The trial is open-label and non-randomized, and its main cancer arms use tumor-response endpoints (RECIST), not polyp counts.

Where the Zolucatetide FAP Data Sits

Earlier FAP drugs were tested in randomized, placebo-controlled trials and published in peer-reviewed journals. Zolucatetide's FAP signal so far comes from company announcements. This table shows the difference.

Drug Evidence type What was reported
Zolucatetide (FOG-001) Company press releases (Mar 5 and Oct 14, 2026); open-label Phase 1/2, no control group Two patients with FAP and desmoid tumors: Spigelman stage IV to a near-normal duodenum, and stage II to stage I. Not peer-reviewed.
Sulindac + erlotinib Randomized, double-blind, placebo-controlled (JAMA 2016), 92 participants Lower duodenal polyp burden than placebo at 6 months; acne-like rash in 87% vs 20% on placebo
Celecoxib Randomized, double-blind, placebo-controlled (NEJM 2000), 77 patients 400 mg twice daily: 28.0% fewer colorectal polyps vs 4.5% on placebo at 6 months
Eflornithine + sulindac Randomized phase 3 (NEJM 2020), 171 patients Disease progression 32% vs 38% (sulindac) vs 40% (eflornithine); not significantly lower

Key Takeaways

✅ What We Know
  • Zolucatetide is an investigational Helicon peptide that Parabilis Medicines describes as a direct inhibitor of the β-catenin:TCF interaction. It is in an open-label Phase 1/2 trial (NCT05919264, drug code FOG-001) that includes an FAP arm.
  • The FAP signal is company-reported. A March 2026 release described one patient with FAP and a desmoid tumor whose duodenal polyposis improved at 60 weeks. An October 2026 release described two such patients: Spigelman stage IV to a near-normal duodenum, and stage II to stage I.
  • The biology is plausible. FAP comes from germline APC variants in the Wnt pathway, and peer-reviewed work since 2012 shows stapled peptides can bind β-catenin directly and block its coactivator role for TCF.
  • Colectomy does not solve FAP. It does not prevent duodenal polyposis, duodenal cancer or desmoid disease, which is why a drug for the duodenum would matter.
  • Earlier drugs reduced polyps in randomized trials (celecoxib; sulindac plus erlotinib), but a 2021 review concluded that no chemoprevention agent had yet met the bar of being effective, safe and tolerable long-term.
⚠️ What We Don't Know
  • Whether zolucatetide works in FAP patients generally. Two patients with no control group cannot show that.
  • Whether the result holds outside patients who also have desmoid tumors. Both reported patients had both conditions.
  • Detailed safety data. The company says no treatment-related serious adverse events or discontinuations have been reported and calls the drug well tolerated, but no adverse-event tables are public.
  • The dose. The new FAP cohort, which opened in October 2026, exists to define the dose and schedule.
  • Whether shrinking duodenal polyps lowers duodenal cancer risk. That is a longer, harder endpoint.
  • Any peer-reviewed efficacy data. As of October 16, 2026, PubMed has no zolucatetide paper.

Frequently Asked Questions

What is zolucatetide?

Zolucatetide is an investigational peptide drug from Parabilis Medicines. It is listed on ClinicalTrials.gov under the code FOG-001. The company describes it as a Helicon (an α-helix-stabilized peptide) that directly blocks the β-catenin:TCF interaction, a step in Wnt signalling that is overactive when the APC gene is defective, as it is in FAP. It is not approved anywhere and is available only inside a clinical trial.

Did zolucatetide reduce polyps in a Phase 1 study?

In two patients, according to the company. Parabilis reported that two people with FAP and desmoid tumors in its open-label Phase 1/2 trial (NCT05919264) had duodenal polyp regression: one from Spigelman stage IV to a near-normal duodenum, the other from stage II to stage I. These are company-reported cases without a control group, and they are not peer-reviewed.

How does zolucatetide differ from celecoxib or sulindac for FAP?

Celecoxib and sulindac are NSAIDs, and their effect on polyps may be related to cyclooxygenase-2 (COX-2) inhibition. Zolucatetide targets β-catenin itself, downstream of the APC defect. The older drugs have randomized trial data, such as a 30.7% drop in colorectal polyp burden with celecoxib 400 mg twice daily at 6 months. Zolucatetide has no randomized FAP data yet.

Can you buy zolucatetide as a research peptide?

Zolucatetide is an investigational drug in a company-sponsored clinical trial. It has no approval, and HighPeptides knows of no legitimate source outside that trial. FAP is managed by specialist hereditary-cancer and GI teams, and anyone with FAP who is interested should ask that team about trial eligibility.

Who is eligible for the zolucatetide FAP study?

Parabilis says the dose-ranging FAP cohort, opened in October 2026, enrolls adults with FAP and a documented APC mutation who have had a colectomy and still have measurable duodenal polyps. Full criteria and sites are on ClinicalTrials.gov under NCT05919264.
