Retatrutide from Veratis, with the testing lab's certificate on every lot

Veratis lists retatrutide as GLP-3 RT. Each lot's certificate of analysis is hosted on the testing lab's own server, so you can check it before you order. Code HIGHPEPTIDES takes 15% off.

🔬 HighPeptides checked the NEJM abstract, not the posts about it. It reports average AHI and weight changes against placebo, and retatrutide beat placebo on every dose. The viral responder and "resolved" percentages do not appear in that abstract, and placebo patients' AHI also fell. Retatrutide is still investigational.

How a Weight-Loss Drug Lowers Apnea Events

⚖️
Weight is a driver of OSA

In a Wisconsin cohort followed over 4 years, a 10% weight loss predicted about a 26% drop in AHI, and a 10% gain predicted about a 32% rise (PMID 11122588).

🧬
Three receptors, large weight loss

Retatrutide activates GIP, GLP-1 and glucagon receptors. In TRIUMPH-1, adults with obesity lost 23.7% (9 mg) and 25.0% (12 mg) of body weight at 80 weeks versus 3.9% on placebo (PMID 42814954).

🌙
Fewer apnea events, more weight lost

In the OSA trials, weight fell 12.5% to 22.2% without diabetes and 12.1% to 18.5% with it, versus 4.8% and 5.6% on placebo. AHI fell more on every retatrutide dose than on placebo (PMID 42904878).

🧪
Built as a basket trial

The OSA studies were nested inside the larger TRIUMPH-1 and TRIUMPH-2 weight trials, each with its own primary endpoint: change in AHI (PMID 41090431).

What the Viral Numbers Leave Out

What went viral What the NEJM paper reports
"Two in three people cut their apnea events by more than half." Despite the social-media framing, the paper's abstract does not report how many people halved their apnea events, so we could not verify this figure. What it does report is average change: without diabetes, AHI fell 34.3 (9 mg) and 31.7 (12 mg) events per hour versus 9.9 on placebo, so placebo patients improved too (PMID 42904878).
"About 1 in 4 saw their sleep apnea resolve." Unverified. The paper's abstract reports AHI and weight changes, not a resolution rate or its definition, so we could not confirm this figure. On the reported averages, the typical participant on the top doses still had moderate OSA at 80 weeks (PMID 42904878).
"A sleep apnea cure." Average AHI fell from 58.6 to roughly 24 to 27 events per hour on the top doses in the non-diabetic trial. That is still in the moderate range for the average participant (PMID 42904878).
"Better than Zepbound." No head-to-head trial exists. SURMOUNT-OSA ran 52 weeks and TRIUMPH-OSA ran 80 weeks, with different baseline AHI, so comparing the two numbers directly is not valid (PMIDs 38912654, 42904878).

Key Takeaways

What We Know
  • In two phase 3 trials, retatrutide lowered AHI more than placebo at 80 weeks, with and without type 2 diabetes (PMID 42904878).
  • Without diabetes, AHI fell 34.3 (9 mg) and 31.7 (12 mg) events per hour versus 9.9 on placebo (PMID 42904878).
  • With type 2 diabetes the effect was smaller: 24.7 and 25.4 events per hour versus 10.3 on placebo (PMID 42904878).
  • Participants lost 12% to 22% of body weight on retatrutide versus about 5% on placebo (PMID 42904878).
  • The most common side effects were gastrointestinal (PMID 42904878).
  • Weight loss alone predicts lower AHI: about 26% lower for a 10% weight loss in a population cohort (PMID 11122588).
What We Don't Know
  • Whether the drop in AHI lowers heart attacks, strokes or deaths. These trials did not measure that.
  • How retatrutide compares with tirzepatide for OSA. No head-to-head trial exists, and the trial lengths differ (52 vs 80 weeks).
  • What happens to apnea after stopping. The trials reported 80-week results, not off-drug follow-up.
  • Whether people already using CPAP can stop. That is a decision for a sleep physician with a repeat sleep study.
  • When or whether retatrutide will be approved. It remains investigational.

Frequently Asked Questions

Does retatrutide cure sleep apnea?

No trial has shown a cure. In the NEJM trials, average AHI in adults without diabetes fell from 58.6 to roughly 24 to 27 events per hour on the 9 mg and 12 mg doses after 80 weeks. That is a large improvement over placebo, but the average participant still had moderate OSA (PMID 42904878).

What did the NEJM retatrutide sleep apnea study find?

Grunstein and colleagues ran two phase 3 trials in 691 adults with moderate-to-severe OSA and a BMI of 27 or higher. At 80 weeks, every retatrutide dose lowered the apnea-hypopnea index more than placebo, both in people without diabetes (TRIUMPH-1-OSA) and with type 2 diabetes (TRIUMPH-2-OSA) (PMID 42904878).

Is it true that 1 in 4 people had their sleep apnea resolve?

We could not verify it. The paper's abstract reports average AHI and weight changes, not a resolution rate. On those averages, AHI in adults without diabetes fell from 58.6 to roughly 24 to 27 events per hour on 9 mg and 12 mg, still moderate OSA (PMID 42904878).

How does retatrutide compare with tirzepatide (Zepbound) for sleep apnea?

They have not been tested against each other. In SURMOUNT-OSA, tirzepatide lowered AHI by 20.0 and 23.8 events per hour more than placebo at 52 weeks (PMID 38912654). Retatrutide trials ran 80 weeks with different baselines, so the numbers cannot be compared directly (PMID 42904878).

Why does a weight-loss drug help obstructive sleep apnea?

Excess weight is a major driver of OSA. In a 4-year population study, a 10% weight loss predicted about a 26% drop in AHI (PMID 11122588). Retatrutide produced 12% to 22% weight loss in the OSA trials, and AHI fell along with it (PMID 42904878).
